At the Doctor's

小明：Hello, I need to see a doctor. I've been feeling unwell lately.

前台：Sure, please fill out this form with your personal information. Have you been here before?

小明：No, it's my first time. Do I need to bring any documents with me?

前台：No, just your identification card would be sufficient. Please take a seat, someone will call you shortly.

医生：Hello, I'm Dr. Smith. What seems to be the problem?

小明：Hi, Dr. Smith. Lately, I've been experiencing frequent headaches and dizziness. I also feel tired all the time.
